WEST COOK COALITION

We believe that individuals with disabilities have the right to envision their own future and to choose the services that best meet their present and future needs. Our goal is to provide personalized services to help our participants and their families live, learn, and work in the community they choose.

As part of the West Cook Coalition (WCC), Community Support Services receives funding through the Illinois Department of Healthcare and Family Services (HFS) as part of the Healthcare Transformation Collaborative. WCC is focused on innovative visions and equity-focused solutions in support of its goal to reimagine healthcare in Illinois.

Through this coalition, CSS employs a Community Healthcare Worker to support individuals with an intellectual/developmental disability (I/DD) and their families. The Community Healthcare Worker will advocate, make referrals, and support individuals with an I/DD and their families in all areas of healthcare including medical, dental, and psychiatric care. Services may be provided one time or as needed on an ongoing basis. CSS offers a continuum of care leading to improved health outcomes for individuals with an I/DD.

Individuals with an I/DD are eligible for these free services if they reside in one of the following 18 communities.

HOW TO APPLY

To qualify for services from Community Support Services, you must be diagnosed with an intellectual/developmental disability and:

  • Reside in western Cook or eastern DuPage county
  • Live in one of the 18 zip codes listed above
  • Fill out a CSS Intake Form
