Funding Request Instructions
The mission of the Proviso Township Mental Health Commission is to provide financial assistance in the form of annual or special grants for operating purposes from funds obtained by assessment of annual tax on real property to agencies involved in providing prevention or treatment services to township residents in the areas of mental health, developmental disabilities, substance use disorder and medical services as they relate to behavioral health.
For New Agency or Program Requests
- Download, complete and submit a Letter of Intent for funding from PTMHC for FY 24-25 by March 31, 2024.
- All agencies and programs that did not receive contracts from PTMHC in FY 24-25 will require a letter of intent before a full application is considered.
- Applications submitted after the March 31st deadline will require a letter of intent (LOI) before consideration.
- Request for financial support after July 1 will only be considered for program expansions and one-time opportunities and will require a letter of intent before a full application can be considered.
- All incomplete applications will not be considered. Electronic submission of all applications and attachments including LOI are preferred.
- Please review our funding guidelines before submitting an application or LOI.

For Current PTMHC Agencies and Programs
- Applications for full-year consideration beginning July 1 are due March 31, 2024.
- To receive a FY 24-25 application please visit the RTS database to download the application or contact Lisa Loster. All completed applications should be uploaded to RTS for consideration.
- Applications submitted after the March 31st deadline will require a letter of intent (LOI) before consideration.
